29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:      a) 27 Wood Lake Drive, Craryville, NY - On or about September 21, 2016 and at times prior thereto, employees were working on a pump jack scaffold at a height of 20'6" using two by four lumber as the scaffold poles.   Employees were not using any type of fall protection while on the scaffold.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:      a) 27 Wood Lake Drive, Craryville, NY - On or about September 21, 2016 and at times prior thereto, employees were washing the siding of the house from a pump jack scaffold at a height of 20'6".  The pump jack scaffold was supported by wooden poles made from two by four lumber.  The poles were not adequately secured, in that there was no bracing at the bottoms of the poles.

29 CFR 1926.452(j)(5): When poles were made of wood, the pole lumber was not straight-grained, free of shakes, large loose or dead knots, and other defects which might impair strength:      a) 27 Wood Lake Drive, Craryville, NY  - On or about September 21, 2016 and at times prior thereto, employees were working on a pump jack scaffold at a height of 20'6" using two by four lumber as the scaffold poles.  The lumber had a number of defects including; knots, cuts, and chips.

29 CFR 1926.452(j)(7): When two by fours were spliced to make a pole, mending plates were not installed at all splices to develop the full strength of the member:      a) 27 Wood Lake Drive, Craryville, NY  - On or about September 21, 2016 and at times prior thereto, employees were working on a pump jack scaffold at a height of 20'6" using two by fours lumber as the scaffold poles.  Mending plates were not installed at any of the splices, only 2 nails were used to connect the two by fours at each end.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:      a) 27 Wood Lake Drive, Craryville, NY - On or about September 21, 2016 and at times prior thereto, employees were working on a pump jack scaffold at a height of 20'6".  Training was not provided to employees to recognize scaffold deficiencies, exposing employees to a fall hazard.
