29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): The employer is failing to protect employees from an engulfment hazard associated with entering a trench without adequate protective systems. The most recent occurrence of this was observed at 6th and Oak St, Greenwood, NE where an employee was observed working in an 8 foot deep trench that was not benched or sloped, nor did it have protective systems in place to protect against sloughing or cave in hazards.
