29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  deleted instance.      b)  Harris Offset Press:  On or about September 22, 2016 and at times prior to; employees operated the press without the use of lower roller guards; exposing operators to crushed-by or pinched-by hazards during regular adjustments and operation of the machine.      * Note  Abatement certification IS required for this item.    The Dixon Container Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.212(a)(1), which was contained in OSHA inspection number 643938, citation number 1, item number 1and was affirmed as a final order on March 19, 2013, with respect to a workplace located at 3210 E. Amity Rd., Boise, Idaho 83716.
