OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to a fall and/or tipping hazard:           (a) Sanctuary: On or about September 22, 2016 an employee working from a Tallescope elevating aerial platform (Model #524) was changing light bulbs in a chandelier at a height of approximately 35 feet. The outriggers were not attached to stabilize and prevent displacement of the unit. An employee fell and was fatally injured when the Tallescope unit tipped over and hit the wooden pews below.                                 ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
