Crossbraces were used as a means of access. a) Exterior of Building - Scaffold South West Facade: Employee was exposed to a fall hazards of approximately 12 feet while engaged in masonry activities from a welded frame scaffold that did not have a proper means of access, on or about 7/9/16.
