29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift: a) Jobsite, 600 North Street - An employee was performing masonry operations from a Skyjack model number SJ40TBOOM 18 feet above ground level without utilizing a personal fall arrest system or personal fall restrain system, observed on or about 6/20/16.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: a) Jobsite, 600 North Street - An employee was performing masonry operations from a Skyjack model number SJ40TBOOM 18 feet above ground level without being trained in scaffold safety. The exposed employee did not know the trigger height for fall protection from extensible booms, on or about 6/20/16.
