Port of New Orleans, Barge #RF0106, starboard side:  On or about September 23, 2016, employee(s) were allowed to operate a Yale forklift #915135, model GDC155VXNJBE112, serial #E879VO1812L, that was not maintained in a safe working order.  Three bolts on the forklift carriage were missing and the locking pins were broken and/or were not secured in the locking unit.

Port of New Orleans, Barge #RF0106, starboard side:  On or about September 23, 2016, employee(s) working under/near a loaded and elevated Yale forklift #915135 were exposed to a struck by/crushing hazard when the left fork blade became dislodged from its carriage and fell striking the employee below.
