29 CFR 1915.111(a): 2 - Defective Gear was not removed and repaired or replaced before further use.    a) Green River Barge Service - On or about 10/4/16, employees were exposed to struck-by hazards when the hook on the hoisting device was missing the safety latch.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
