29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      Construction Site: On or about October 04, 2016, two employees working on a steep roof without any form of fall protection. This condition exposed the employees to a fall hazard of more than 6-feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    Construction Site: On or about October 4, 2016 and at times prior thereto, one employee was using an A-Frame ladder leaning against the wall to gain access to an upper level of a residence. This condition exposed the employees to a fall hazard of more than 6-feet to the ground below.
