29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Location:  Machine # 2    Instance A:  A guard along the front of the first dryer section did not prevent an employee's arm from being caught by the roller felt and pulled into the machine.  The employee was seriously injured.    Instance B:  In-running nip points between the felt rollers and the Drying Cans are unguarded.    Instance C:  An in-running nip point between the paper and a guide roller just before the paper enters the Farrel roll.     Instance D:  In-running nip point created by the horn roll and the shaft it drives.    Instance E:  Wall mounted emergency stops were located where they were not readily accessible.    Instance F:  Multiple emergency stops were guarded in a manner that impeded their access.
