29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Jobsite:  An employee was operating a Kubota RTV 1100 on a narrow access path with a folded stepladder protruding from the side that struck and injured an employee.  The operator was not trained in the recognition and avoidance of the hazards associated with operating the Kubota RTV 1100 in this manner.
