29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of job-sites, materials, and equipment to be made by a competent person(s):     (a)  Foundation 2 Finish,LLC, 7160 E. Oxford Avenue, Denver, CO:  On or about September 28th, 2016, the controlling employer in charge of the worksite did not ensure a "competent" person made frequent inspections of the job-site to ensure the use of fall protection while employees working for a sub-contractor removed wood shake shingles. Two employees working for a roofing sub-contractor were observed tearing off wood shake shingles on a 7/12 slope single story residence without Personal Fall Arrest Systems (PFAS).  This condition exposed employees to a fall hazard of approximately 9 feet to the ground below.
