29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel. a) Jobsite: Yard - A powered industrial truck operator, who was assisting an employee load lumber and building materials onto a 2004 Volvo tri-axle straight truck, with a 11,000 lb. capacity Hyster Fortis powered industrial truck weighing 17,150 lbs., traveled in a rearward direction towards the employee standing behind him without looking in the direction of, and keeping a clear view of the path of travel, striking and pinning the employee underneath the powered industrial truck. The employee suffered a leg amputation above the knee as a result. Noted on or about 9/26/16.
