On or about October 5, 2016 employees engaged in residential roofing activities at heights ranging from approximately nine (9) to eighteen (18) feet, were not protected by any type of fall arrest system.

On or about October 5, 2016 employees engaged in residential roofing activities at heights ranging from approximately nine (9) to eighteen (18) feet, were using 2 x 4's/2 x 6's and roof jacks as a substitute for an adequate fall protection system.
