In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The outlet box being used was designed to installed on a ridged surface only. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:(a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY) On or about October 5, 2016: a) Employees were exposed to electrical shock and or burns while utilizing a metal outlet box with pre-punched knockouts as an extension cord to provide power to equipment such as but not limited to extension cords and power tools. The outer insulation of flexible cord entering the box had come away from its fitting, exposing the insulated conductors inside. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 5, 2016: a) Employees were exposed to fall hazards of approximately 16 feet while installing roof sheathing on a lowslope roofing system without the use of fall protection measures. Dream State Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1110686, citation number 1, item number 1 and was affirmed as a final order on February 2, 2016, with respect to a workplace located at 1530 Mercer University Drive, Macon, Georgia. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
