29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     On the east side of the construction site, employees engaged in roofing activities used pneumatic nail guns and were not wearing eye and face protection exposing them to struck-by hazards and eye injuries.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   On the east side of the construction site, employees engaged in roofing activities on a pitch roof of 10:12 without fall protection were exposed to a fall hazard of approximately 30 feet to the ground below.
