On or about 10/12/2016 at the Moss Creek Estates lot 51, Alpharetta, GA:  JLNC, Inc., the creating and correcting employer who was responsible for assembling, disassembling, and operating the crane failed to protect employees from electrical hazards associated with crane equipment operating within 20 feet of a 119.15 kV power line that was located within the maxim working radius of the crane by de-energizing and grounding  the lines, ensuring that no part of the cranes equipment operated within 20 feet of the power lines or determining and maintain the minimum approach distance of 15 feet specified for a nominal line voltage of 119 kV in Table A. One employee was fatally injured and another employee was seriously injured from the crane coming closer than 20 feet or without de-energizing the overhead power lines in the area.
