29 CFR 1926.1408(a)(2): The employer did not determine if any part of the equipment, load line or load (including rigging and lifting accessories), or met the requirements in Option (1), Option (2), or Option (3) of this section, if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line. On or about 10/12/2016 at the Moss Creek Estates lot 51, Alpharetta, GA: JP General Construction, LLC, a Framing Contractor was hired to frame homes in the Moss Creek Estate subdivision and at lot 51 where the employer failed to protect the employees engaged in truss connecting and lifting from the electrical hazards by de-energizing and/or ensuring that no part of the crane, rigging, and lifting equipment got closer than 20 feet from the live overhead power lines rated at about 119.15KV. One employee was fatally injured and another employee was seriously injured from the crane coming closer than 20 feet or without de-energizing the overhead power lines in the area.

29 CFR 1926.1408(g)(1)(i): The employer had not train the each employee and crew member assigned to work with the equipment in the procedures to be followed in the event of electrical contact with a power line. Such training must include: 1926.1408(g)(1)(i) A through F. On or about 10/12/2016 at the Moss Creek Estates lot 51, Alpharetta, GA: At this job site, employees engaged in and preparing to elevate wooden trusses and other wooden material with the Manitex Hydraulic Boom Truck Crane was exposed to an electrical contact while working near the live overhead power lines rated at about 119.15KV. The employer had not trained the each operator or crew member on the procedures to be followed in the event of electrical contact with a power line. On 10/12/2016 one employee was fatality injured and another employee was seriously injured when the crane equipment including rigging equipment came in contact with the live overhead power lines.
