29 CFR 1926.50(g): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. a. On or before October 12, 2016 where an employee mixed mortar containing Portland cement, an eye wash station was not provided, thereby exposing the employee to a contact hazard. b. On or before October 12, 2016 where employees used mortar containing Portland cement, an eye wash station was not provided on the same level where the employees were working, thereby exposing the employees to a contact hazard.

29 CFR 1926.51(f)(1): The employer shall provide adequate washing facilities for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be in near proximity to the worksite and shall be so equipped as to enable employees to remove such substances. a. On or before October 12, 2016, where employees were using mortar containing Portland cement, clean water, non-alkaline soap, and clean towels were not provided, thereby exposing the employees to contact hazards.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold shall be protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity). a. On or before October 12, 2016, where employees were working from a scaffold, the employees were not protected from falls from the open side and ends, therefore being exposed to a 16 foot fall hazard.

On or before October 12, 2016, where employees were working from a fabricated frame scaffold, a mud trough was placed on side brackets on the outside of the scaffold, exposing the employee below to crushed-by or other hazards.
