29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a. On or before October 12, 2016, where employees were working from the third level of a fabricated frame scaffold, guardrail were not installed along all the open side or ends of the scaffold, thereby exposing employees to a 16 foot fall hazard.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a. On or before October 12, 2016, where employees were installing a slate roof, the employees were not protected from falls, thereby being exposed to a 18 foot fall hazard.
