29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) On or about 10/12/2016 - Production area, injection molding machine, Milicron, Model SF1000MWP-2X830, S/N N5700AA020003, 7049 Greenbrier Parkway NW, Madison, Alabama, employer allowed employees to remove injection nozzle from the injection molding machine without the use of appropriate face protection exposing them to molten resin when stored energy was not controlled.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) On or about 10/12/2016 - Production area, injection molding machine, Milicron, Model SF1000MWP-2X830, S/N N5700AA020003, 7049 Greenbrier Parkway NW, Madison, AL, the employer did not develop or utilize Lockout/Tag out procedures when employees were removing the nozzle of the injection molding machine, to ensure the pressure relief valve did not have stored energy.
