29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) On or about October 7, 2016, employees were exposed to fall hazards while roofing a house without fall protection. The fall hazards were 9.8 feet above the ground and greater. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii): (a) On or about October 7, 2016, employees who were exposed to fall hazards while reroofing a house, were not trained in the use and operation of personal fall arrest systems. Three of the six workers on site had personal fall arrest systems, and the three with the systems were wearing the harnesses too loosely and without all of the buckles secured. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
