29 CFR 1926.501(b)(13):  "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 10/03/2016, employees from Yoni Perez were replacing shingles on the roof of a residence located at 1094 Treeshade Drive, St. Peters, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The crew leader and five employees were observed on the roof of the building approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.1053(b)(1):   When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 10/03/2016, employees from Yoni Perez were replacing shingles on the roof of a residence located at 1094 Treeshade Drive St. Peters, MO.  The ladder the employees were using to access the roof was not extended three feet beyond the landing or edge of the roof.
