29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the employer without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals were not be changed accordingly:  Assembly Area:  On, or about, October 13, 2016, the identification plate provided on the CAT forktruck listed a 24 inch load center.  The forktruck was provided with 5 foot long forks.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition:  Assembly Area:  On, or about, October 13, 2016, the following deficiencies were found on powered industrial trucks:  A.   The CAT forktruck was provided with a cardboard weather cover that obstructed the operators view through the overhead guard.  B.  The Clark forktruck was emitting an excessive amount of carbon monoxide.
