29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  During work on the clubhouse, as part of the apartment complex at about 16333 Truman Rd., Ellisville, MO, where work was performed on the building front overhang, from a work platform supported and raised by a JCB rough terrain forklift, the platform was not fully secured to the equipment.  One of two pins was missing to secure the platform to the forks and a chain was not used for additional securing (the pin was missing from the end of a chain used to ensure that the pin was always attached to the platform and available for securing to a fork).  Both the pins and the chain were required by the platform manufacturer to secure it to the forklift.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:  During work on the clubhouse, as part of the apartment complex at about 16333 Truman Rd., Ellisville, MO, where work was performed on the building front overhang, from a work platform supported and raised by a JCB rough terrain forklift, the platform was missing midrail protection because of damage to the platform railing.  (Note: further damage was to the top rail which was displaced by about 1.5 ft, a damaged vertical support, and a damaged swinging side gate that was not latched because of the damage.)
