29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a. On or about 10/11/2016, at the site - an employee using the aerial lift which had an arm mounted winch while inside the bucket was ejected from the bucket after the line attached to the winch broke and caused a catapult effect. The employee fell about 33 feet 2 inches to a concrete sidewalk, in that, personal fall protection was not used.
