29 CFR 1926.451(c)(2)(v):   The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  On or about October 3, 2016, at or near 4276 Highway 51, LaPlace, LA; the employer used a forklift to support a scaffold platform and allowed the forklift to be moved horizontally while the platform was occupied.

29 CFR 1926.451(g)(1)(vii):   Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  On or about October 3, 2016, at or near 4276 Highway 51, LaPlace, LA; each employee in an elevated scaffold platform attached to a forklift was not protected from falling approximately 20 feet to the ground below by use of a personal fall arrest system or guardrail system.
