29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Front of Building, Second Floor - On or about October 11, 2016, masons were exposed to a 18 foot fall from an unguarded open-sided floor which had not been provided with standard guardrails, nets or other means of personal fall protection. b) First Floor - On or about October 11, 2016, workers were exposed to an 8 foot 7 inch fall at an unguarded open-sided floor. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) Second Floor - On or about October 11, 2016, workers were exposed to a 20 foot fall from a 38 inch wide wall opening which was not provided with fall protection, including, but not limited to, personal fall protection systems, nets, or standard guardrails. b) Second Floor - On or about October 11, 2016, workers were exposed to a 20 foot fall from a 7 foot wide wall opening which was not provided with fall protection, including, but not limited to, personal fall protection systems, nets, or standard guardrails. c ) Second Floor - On or about October 11, 2016, workers were exposed to an 18 foot fall from a 52 inch wide wall opening which was not provided with fall protection, including, but not limited to, personal fall protection systems, nets, or standard guardrails. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
