29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  a) 854 N 12th ST - On or about 10/17/2016 the employer allowed the employees to use an energized flexible extension cord which was worn, frayed and missing sections of the outer sheath thus exposing the employees to electrical hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:  a) 854 N 12th ST:  On or about 10/17/2016 employees performing roofing operations (4 in 12 pitch) approximately 20 feet above the ground without the use of a fall protection system thus exposing the employees to fall hazards.
