29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    On or about October 18, 2016 and at times prior thereto, employees were observed throwing roofing debris into a dumpster approximately 30 feet below while another employee on the ground was picking up the debris without wearing head protection, exposing the employee to the hazard of being struck by falling materials.

On or about 18 October 2016 and at times prior thereto, four employees without eye protection were using pneumatic nail guns to secure composition shingles to the roof of the apartment complex, exposing the employees to the hazard of being struck by flying objects.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed;   a) On or about 14 October 2016 and at times prior thereto, eight employees were using a 24 foot extension ladder, which was not extended three feet above the upper landing, to access the third story roof of an apartment complex to conduct re-roofing activities, exposing the employees to the hazard of falling approximately 30 feet to the ground below.  b) On or about 18 October 2016 and at times prior thereto, eight employees were using a 40 foot extension ladder, which was not extended three feet above the upper landing, to access the third story of an apartment complex to conduct re-roofing activities, exposing the employees to the hazard of falling approximately 30 feet to the ground below.

On or about 14 October 2016 and at times prior thereto, eight employees were using a 24 foot extension ladder, which had a horizontal distance greater than approximately one-quarter of the working length of the ladder, to access the third story roof of an apartment complex, exposing the employees to the hazard of falling approximately 30 feet to the ground below.
