29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented. At the new residential construction site, employees installing roofing material without fall protection were exposed to fall hazards of approximately 18 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the new residential construction site, employees climbed ladders that were not extended at least 3 feet above the upper landing to access and egress the roof area were exposed to fall hazards.
