29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     The employer is failing to protect employees from fall hazards associated with roofing without adequate fall protection.  The most recent occurrence of this was observed at 8611 Oakmont St, Lincoln, Omaha, NE 68526 where employees were observed installing composite asphalt shingles on a roof that was approximately 10' high and they were not utilizing fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    The employer is failing to protect employees from a fall hazard by not providing fall hazard training to employees.  This was most recently observed at 8611 Oakmont St, Lincoln NE 68526 where employees were installing composite asphalt shingles on a roof that was approximately 10' above ground and the employees had not received fall hazard training.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    The employer is failing to protect employees from a fall hazard associated with using a ladder that does not extend at least 3 feet above the upper landing surface. This was most recently observed at 8611 Oakmont St, Lincoln, NE, 68526, where employees utilized an extension ladder to ascend a 10' roof and the ladder only extended approximately 2' above the upper landing surface.
