29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)Phil Built, LLC @ 4500 S Oxbow, Sioux Falls, SD 57106: On or about October 18, 2016, employees were exposed to injuries from falls, while conducting re-roofing activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface. The condition exposed employees to a fall hazard of approximately 17 feet.

Oxbow, Sioux Falls SD, 57106:  On or about October 18, 2016, employer failed to provide employees with training to recognize hazards associated with working from heights.
