Paul Tirpak dba Tirpak Building Maintenance, 2401 West 38th Street, Erie, PA 16506, upper roof of the structure, on or about October 19, 2016 and times prior thereto: Employees were accessing the upper roof of the structure to access a ladder that would lead them to a scaffold platform where work was taking place.  A means or method of fall protection was not employed to completely protect the employees when accessing the upper roof.  Employees were exposed to a fall hazard of 24.6-feet to the lower roof below.
