29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation: a) At 1901 East Glenn Ave., Auburn, Alabama: On or about October 19, 2016 and at times prior; the employer allowed his employee to work from a two buck high scaffold which did not have base plates installed to prevent damage to the scaffolding exposing his employees to a fall hazard of 12 feet 11 inches to the ground below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) At 1901 East Glenn Ave., Auburn, Alabama: On or about October 19, 2016 and at times prior: the employer allowed his employees to use the scaffold side as a ladder to gain access to the work platform and the sides of the scaffold are not designed to be used as a ladder exposing his employee to a fall hazard of 12 feet 11 inches to the ground below.
