Employees were standing on two pallets screwed together as a platform, that was elevated by a Caterpillar Tele-Handler. Employees were not wearing fall protection during this activity. The platform was not designed by a qualified person, and had been assembled on site. NOTE: Abatement certification and documentation are required for this item.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows: a) On or about October 11, 2016 or prior thereto: Employees were installing a metal hood for a HVAC fan at a height of approximately 25 feet from ground level. Employees were standing on two pallets screwed together as a platform, that was elevated by a Caterpillar Tele-Handler. Employees were not wearing fall protection during this activity. The platform represented a slip, trip and fall hazard as it was not fully planked. There were not uprights or guardrails for this platform. NOTE: Abatement certification and documentation are required for this item.
