29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:   a) Job site, 7300 Cranberry Springs Drive, Cranberry Township, PA: On or about October 19, 2016, during gravel leveling activities in an excavation, the employer did not ensure that employees had trench and excavation safety training in that employees were working in an excavation that did not have adequate cave in protection in use, exposing them to being caught in an excavation collapse. The excavation depth was determined to be between approximately 8 and 15 feet and had wall angles primarily around approximately 70 to 80 degrees.
