29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       (a) Hometown Remodeling, LLC @ 6400 Callington Circle, Sioux Falls, SD 57108: On and prior to October 18th, 2016, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in re-roofing activities while working at the unprotected edges of a 6 in 12 pitched roof on a single-family home. The condition exposed employees to a fall hazard of approximately 9 feet.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     (a) Hometown Remodeling LLC @ 6400 Callington Cir., Sioux Falls SD, 57104:  On or about October 18, 2016, the exposing employer did not ensure that a portable extension ladder, used to access the roof, had side rails extending 3 feet above the upper landing, or was secured. The condition exposed employees to a fall hazard of approximately nine feet.

29 CFR 1926.1053(b)(20): Employees did not face the ladder when ascending or descending the ladder:  (a) Hometown Remodeling LLC at 6400 Callington Cir., Sioux Falls SD, 57104:  On or about October 18, 2016, employees were exposed to injuries resulting from falls, while descending a portable ladder.  Employees were descending a portable ladder facing away from the ladder which exposed the employees to a potential fall distance of 9 feet.
