On or about October 14, 2016, at the west side of the residential building, an employee using an air nail gun to install wood bracing on the roof was not wearing eye protection, and exposed to flying debris and struck-by hazards.

On or about October 14, 2016, at the west side of the residential building, an employee doing wood bracing work while standing on the top plate at the leading edge was not protected from a 20 feet fall hazard by the use of a fall protection system. b. On or about October 14, 2016, at the east side of the residential building, two employees doing sheathing work on a 4:12 pitch residential roof were not protected from a 10 feet fall hazard by the use of a fall protection system.

On or about October 14, 2016, at the front side of the residential building, an employee doing roof sheathing work on a 4:12 pitch roof had his personal fall arrest system rigged inappropriately and did not prevent him from falling more than 6 feet, exposing to a 15 feet fall hazard.
