29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about October 14, 2016, at the west side of the residential building, an employee using an air nail gun to install wood bracing on the roof was not wearing eye protection, and exposed to flying debris and struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about October 14, 2016, at the west side of the residential building, an employee doing wood bracing work while standing on the top plate at the leading edge was not protected from a 20 feet fall hazard by the use of a fall protection system.     b. On or about October 14, 2016, at the east side of the residential building, two employees doing sheathing work on a 4:12 pitch residential roof were not protected from a 10 feet fall hazard by the use of a fall protection system.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a.  On or about October 14, 2016, at the front side of the residential building, an employee doing roof sheathing work on a 4:12 pitch roof had his personal fall arrest system rigged inappropriately and did not prevent him from falling more than 6 feet, exposing to a 15 feet fall hazard.
