29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: a) 2324-28 Memphis St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 28 feet long, 39 feet high and 5 feet wide which was not fully planked or decked on all working levels between the front uprights and the guardrail supports. This hazard exposed the employees to a fall hazard ranging from approximately 13 to 20 feet. This condition was observed on October 20, 2016.
