29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:    a) 2324-28 Memphis St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 28 feet long, 39 feet high and 5 feet wide which was not fully planked or decked on all working levels between the front uprights and the guardrail supports. This hazard exposed the employees to a fall hazard ranging from approximately 13 to 20 feet. This condition was observed on October 20, 2016.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames and uprights were not bear firm foundation:    a) 2324-28 Memphis St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 28 feet long, 39 feet high and 5 feet wide which did not bear on a firm foundation. This hazard exposed the employees to a scaffold collapse hazard. This condition was observed on October 20, 2016.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level by use of personal fall arrest systems or guardrail systems:    a) 2324-28 Memphis St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 28 feet long, 39 feet high and 5 feet wide which did not have a guardrail system installed on the working levels of the scaffold. This hazard exposed the employees to a fall hazard ranging from approximately 13 to 20 feet. This condition was observed on October 20, 2016.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers or stairway-type ladders was not used for access:    a) 2324-28 Memphis St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 28 feet long, 39 feet high and 5 feet wide which did not have proper access for the employees to reach the various working levels of the scaffold. This hazard exposed the employees to a fall hazard. This condition was observed on October 20, 2016.
