29 CFR 1910.269(v)(11)(xii): Where coal-handling operations may produce a combustible atmosphere from fuel sources or from flammable gases or dust, sources of ignition shall be eliminated or safely controlled to prevent ignition of the combustible atmosphere:    (a) Facility, 243 Rubisch Road, Ebensburg, PA:  On or about October 4, 2016 the employer did not eliminate or control an ignition source when having maintenance performed in the refuse fuel bunker.  The employer did not ensure that the area had been wet down or other cleaning methods conducted to reduce any dust that may shake up and create a combustible atmosphere. A contractor used explosive materials in an attempt to loosen material stuck inside the section of the refuse bin and an explosion occurred.
