(a) Location - 360 Lowell Street, Lawrence, MA: On or about 10/24/2016, an employee was allowed to use an extension ladder with damaged rungs.

(a) Location - 360 Lowell Street, Lawrence, MA: On or about 10/24/2016, the employer did not ensure employees who were assigned to utilize ladders were trained to recognize hazards and understand ladder procedures associated with ladder use.
