1926.1060(a)(1):  The employer did not ensure that each employee was trained by a competent person which would enable each employee to recognize hazards related to ladders and stairs, and the procedures to follow to minimize those hazards:  On or about October 21, 2016, at the above addressed job site, the employer did not did not ensure that each employee was trained by a competent person to enable each employee to recognize hazards related to ladders, and the procedures to follow to minimize those hazards as employees had been using a step ladder in the closed position to enter and exit a trench while performing repairs to a water valve.
