The travel distance to the closest fire extinguisher was 819-feet.

JH Waters, Inc., 2601 Sigsbee Street, Erie, PA  16508, east side of reservoir perimeter sidewalk, on or about October 26, 2016 and times prior thereto:  Flexible electrical extension cords had damage including, but not limited to, melted cords caps and fraying of the outer jackets.  The employer did not identified the flexible electrical extension cords as unsafe by tagging the equipment out of service, rendering them inoperable, or physically removing them from the worksite.
