29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) On or about October 26, 2016, or prior thereto:  Oil line maintenance work was conducted on lines approximately 12.5 feet above ground level, in the path of a motorized garage door.   The garage door was not disconnected prior to the commencement of the maintenance work.    The employer does not have an energy control program to prevent unexpected energization of equipment.  Note:  Abatement certification is required for this item.
