29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On or about 10/25/2016 - 7340 Vidette Lane, Lot #19, Owens Cross Roads, AL, employees gaining access to the roof were using ladders that were not extended at least 3 feet above the landing surface.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems:    a) On or about 10/25/2016 - 7340 Vidette Lane, Lot #19, Owens Cross Roads, AL, employees installing asphalt shingles on a steep-sloped (9:12) roof were not protected from falls of approximately 9 feet to the ground.      Mighty Rock Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 905783, citation number 1, item number 2 and was affirmed as a final order on 06/06/2013, with respect to a workplace located at 115 Chesnut Heath Court, Madison, AL 35756.
