OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from deficiencies with the overhead hoists (underhung):    (a) Facility, 1 Industrial Park Drive, Bay B, Oakdale, PA:  On or about October 27, 2016 the employer permitted employees to use the overhead hoist that had the following deficiencies:  the brakes on the Acco Wright Speedway 5 ton hoist did not stop and hold the load when the controls were released.  The hoist continued to travel up or down approximately 3-5 inches when the controls were released.        (b) Facility, 1 Industrial Park Drive, Bay A, Oakdale, PA:  On or about October 27, 2016 the control pendant on the A2 Acco Wright 7 1/2 ton hoist had a broken case exposing the operator to potential electrical shocks.
