OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from deficiencies with the overhead hoists (underhung):    (a) Facility, 1 Industrial Park Drive, Bay B, Oakdale, PA:  On or about October 27, 2016 the employer permitted employees to use the overhead hoist that had the following deficiencies:  the brakes on the Acco Wright Speedway 5 ton hoist did not stop and hold the load when the controls were released.  The hoist continued to travel up or down approximately 3-5 inches when the controls were released.        (b) Facility, 1 Industrial Park Drive, Bay A, Oakdale, PA:  On or about October 27, 2016 the control pendant on the A2 Acco Wright 7 1/2 ton hoist had a broken case exposing the operator to potential electrical shocks.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:  (a) Facility, 1 Industrial Park Drive, A bay, Oakdale, PA;  On or about October 27, 2016 a flexible cord being used to power a spray paint machine had several cuts and slices in the insulation and was not continuous in length without splice.  (b) Facility, 1 Industrial Park Drive, B bay, Oakdale, PA:  On or about October 27, 2016 a flexible cord used to power grinders and other power tools had several cuts and slices in the insulation and was not continuous in length without splice.

29 CFR 1910.184(d): Sling(s) and fastenings were not inspected each day or during use and were not removed from service when damaged or defective:  (a) Facility, 1 Industrial Park Drive, Oakdale, PA:  On or about October 27, 2016 synthetic and chain slings were not inspected each day or removed from service when damaged or defective.  Employees were using synthetic slings with severe cuts that had not been removed from service.  Employees used chain slings that did not have tags to ensure employees used the proper sling for the weight of the load being lifted.
