29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  Jobsite:  Tubular welded frame scaffolding was not erected by trained and experienced employees under the supervision of a competent person.  The employees erected tubular welded frame scaffolding that was not fully planked, did not have mud sills, and did not have access ladders.  The employees also erected a ladder supported scaffold by placing a board on the rungs of an aluminum extension ladder and a fiberglass stepladder.  This exposed the employees to fall and struck-by hazards.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  Jobsite:  Employees placed a  dimensional lumber board on the ninth (9) rung of an aluminum extension ladder and the top step of a fiberglass stepladder and were using it as a work platform.  This exposed the employees to falls of approximately nine (9) feet to the concrete below.
