29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about October 11, 2016 on the East side of the building, the employer allowed two employee(s) to utilize an aerial lift(JLG Model 800-A) without adequate fall protection while installing plywood sheeting on the exterior windows of the building at a height of approximately thirty-two (32) feet.
