Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (A) On or about June 27, 2016 at the above addressed jobsite, employees were walking on the deck of the structure to access the deck of the hopper to perform work. Employees were exposed to a fall hazard of approximately 15 feet into the Illinois River due to an inadequate guardrail system.   (B) On or about June 27, 2016 at the above addressed jobsite, an employee was working on a conveyor system located in the dry fertilizer building and was not utilizing any means of fall protection. The employee was exposed to a fall hazard of approximately 11 feet.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
